Achievement "Won Them All" not unlocking. I have all 7 game mode winning ribbons. XBOX360. What is wrong?

I have won all 7 game modes and have all 7 winning ribbons.

Modes are:

Conquest
Rush
Team Death Match
Squad Death Match
Domination
Defuse
Obliteration

Why has this achievement not unlocked?

on 3rd party websites this achievment appears to be a multiplayer one.  is it actually a single player one?  and I have to win the game in easy/medium and hard modes?  Or is this multiplayer achievement and its broken for me?

I've won defuse but get no xp or ribbons. Every other game mode works for me. I'm trying to get the 'won them all' trophy too
