I spent a very frustrating couple of days a month or two ago trying to figure out what I was doing wrong. When they added the form it made the extra box mandatory for some reason, despite it being for additional information. Saw someone mention it on some other post or I never would have figured it out on my own. Glad you asked and got it resolved faster than I did lol
