Steps: How can we find the bug ourselves? Play a child ghost given from the wishing well in Romantic Garden pack. (Most negative moodlet for Wishing Well then ask for a child, will be given a ghost child)
What happens when the bug occurs? My ghost child is a ghost in CAS, on Main Menu Load Game photo, and Manage Worlds photo. When I actually play her in-game child is alive. She can't possess anything and has her color. This happened right after my game uploaded the new ghost update. Didn't have any problems prior. My guess is the new ghost update didn't account for prior made Wishing-Well-granted ghost children.
What do you expect to see? My ghost sim acting as a ghost, transparent, floating, and possessing.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Yes
Please describe the patch or change you made. The New Sims 4 update for Ghosts.

I had this issue. They were death by Laughter from the well in my game. For now everyone who has this issue a work around with cheats. Give them either drowning or fire with "cas.fulleditmode" after using "testingcheats on" since those are the only ghost traits they can have. This wishing well hasn't been updated with the redone ghosts. It's bugged at the moment. Console players I understand you don't want to turn on cheats. Right now this is the only work around I have found.

Steps: How can we find the bug ourselves? Loading the game with a ghost in the household.
What happens when the bug occurs? I have a ghost child who was from the Romantic Garden wishing well and who had a death of laughter. When I loaded in to the game, I went to CAS and could edit them through the new updates and the death was still there. The sim still appeared as a ghost in the family preview. When I load into the household to play, the sim is no longer dead and lacks a milestone for their death by laughter. When I load from their to CAS, the sim acts as a regular sim. I closed the game without saving, reloaded, and the sim was a ghost from the household family preview. I loaded into the house and the sim was again marked as alive. Every time I load into the house to play when starting fresh, the sim acts as a living sim despite having existed prior as a ghost.
What do you expect to see? The ghost sim should appear as a ghost, since they are dead and have not been resurrected.

I do not have a script mod installed and was not using any CC clothes on the sim. The only CC I have downloaded are decor items, which are not present in the lot I loaded in this issue. The sim does not have a gravestone since they were generated from the Romantic Garden wishing well and has a Death by Laughter. Attached are the images that show that she was dead and now is not without resurrection. I have not saved my progress so when I reload the sim is still a ghost. I noticed in CAS when starting a new household, Death by Laughter is not an option for child sims.

I have the exact same issue for the exact same reason! I don't have any mods/cc in my game at the moment however. But my ghost child who was adopted from the wishing well, with a death by laughter, keeps becoming human. Luckily I have the save backed up, but as I want him to be a ghost and remain as is, this is making my main sims save pretty much unplayable.

Ensure that it will result in the 'Death outcome.'
Select the "Wish for a child" option.

For everyone clicking me too, did you also get the ghost through the wishing well ? What death type ?

My guess is that the wishing well gives death types they no longer allow for children, hence the conflict.

The death type is death by laughter. My child shows this death type in CAS but not in play mode.

Thank you! I changed the death type to Death by Fire and my child sim is a ghost again. I'm surprised that death by fire or death by water are the only two options for children. It feels more morbid than death by laughter. I'm going to miss my ghost child getting playful moodlets due to her death, she could make other Sims playful too and really added too my storyline. 😭

I'm having this issue too! Although admittedly I did a little finagling with cheats to give them a different death type a few weeks ago. I had given them the Death by Urban Myth after summoning wishing for them from the Well and it was perfectly fine before the patch, and then the patch brought them back to life!! The only deaths that I could give them back in CAS was fire or drowning and it makes me so sad, I had a whole story line going

I had this problem as well I have the game on 2 machines (all packs)

game with no mods i had 2 ghost children and on the mod game 1 ghost child.

now all human again  I was going to wait for new pack then resurrect them 

Now all future plans for them is ruined and i am sad.

I remember one death was by laughter they were funny and made others happy.
