Can you give this a shot for me to see if it helps? 

1. Open up your Xbox Settings

2. Go to Network

3. Select Advanced Options

4. Choose the option to clear the Alternate MAC Address

The Xbox will restart after that. Let me know if you're still running into this same issue afterward. Thanks!

I been having the issue on the PS4 now and it also says an unknown issue.

Do you have Madden NFL 21 installed on external hard drive, or is it stored on the drive that's built into your PlayStation?

Would it be possible to try moving it to the internal drive to see if that helps at all? I've seen some cases where issues like this can come up when the game is installed on an external drive. If that's something you can try please let me know how it goes. Thanks!
