Those worst times you have are a problem. While the average is fine, it indicates high ping spikes and that will cause lag/disconnects. I recommend you contact your ISP as this appears to be starting with your modem/router. They can perform some testing to see if there are any issues with it.

That 2nd hop is really rough. That's the node in your neighborhood. Which might be getting overloaded if you live in a densely populated area. You also have high ping times when you hit the 15th hop. I wouldn't be terribly concerned about that as it's possible that's due to the distance between you and the server you're pining. My main concern is that 2nd hop.

 

Did you make any changes? This is a lot different than your last test.

That connection looks better than the previous one, but it's still going to cause a lot of lag. A VPN is probably going to cause lag as well.
