Steps: How can we find the bug ourselves? With Jaal in the party, complete "Contagion" without having first finished "Jaal Ama Derav: Friend or Foe?". Take the option to kill the Roekaar leader, read the datapad about how they have the ultimate weapon blah blah blah, and listen to the companion commentary.

Companion script bug in "Contagion".  I collected and completed "Contagion" with Jaal in the party but without having completed his loyalty mission, "Jaal Ama Derav: Friend or Foe?" and he made a side comment after I read the Roekaar leader's datapad about them having a "weapon to destroy the interlopers" that didn't make sense in context.  He said something about Akksul being dead, but having not completed his loyalty mission I haven't even met Akksul yet, let alone killed him.

I have experienced this too and I had finished Jaal's loyalty mission.

Not to be too spoiler-y, but to add further confusion caused by this, Akksul was alive by the end of the mission and I even got an email from him later. At first, I thought maybe something had happened off-screen after the mission, but no, he should still be alive, so where the "Akksul's dead" comment is coming from makes no sense in this case.
