Hi,
I have been suffering from the same lag spike issues, but my ms can vary from 161 which is normal upto what i have screenshot with my mobile which happened only a few moments ago. It got to about 536,516ms before i finally traveled to my Sky Palace from Rishi without a disconnection. Now if it disconnected me then i know it is my end and that is a rare occurrence.
I have noticed these huge lag spikes have only started recently since the last launcher update. It would be great if this were fixed pronto.
One thing is when the lag spikes happen the only things to lag when i go to google to search for a issue is anything related to swtor.com or EA's site. So it is clearly not my end here.
Any help or ideas would be great but i think it is all server side to be honest.

What AV ?, Maybe remove and re-add exceptions for launcher.exe and swtor.exe to the AV and firewall
swtor.com is hosted on Akamai servers so nothing to do with the game servers
eg I am In Australia (see spoiler)

I use free Avast as my AV. Sorry for the slow reply. I am also an aussie east coast. I only ever have lag issues in swtor not any other game.

I experienced this sporadically since Galactic Strongholds launched, specifically on Harbinger while other servers were fine. When I get the sky high ping (which is rare luckily nowadays) simply logging out and logging back into my toon (I don''t have to log out of the game) usually brings my normal ping back without lag.

If you use the free trial of www.wtfast.com does it stabilize ?
The Consistent Jitter is interesting in that it is consistent once you cross the international route, worth keeping eye on.
Whilst it doesn't appear to be affected too badly the consistent pattern is a classic sign

Will mention the packet loss to @EA_Nils seen at the final destination in case it is an issue, I am not seeing it to that IP but I have a different route

That one also points to an issue with the international link between Sydney and San Jose
Edit Closest to that is the farend bandwidth congestion listed here just yours isn't far end its more near end
