This happens sometimes on PC too were a command is not carried out or dropped in the midst of processing by the system or game engine. Only for me this happens not too often more like once every one/two weeks or so.
What you can try is:
-Make sure the button is not faulty and pressed completely
-Check heat of system is not too high and all is clean and clear of obstructions (also the bottom of the machine should be resting on the stands only)
-Test wired cable internet connection and disable WiFi as test and reset router to defaults before trying
-Your ingame player ping must not be too high (under 80ms)
-Test smaller serrvers with less players and action to see if this makes thing any better
-Test muti and single player game modes
-Test other games
