DxDiag - Controller on PC Issue

Is there a reason why whenever I try to use my controller on my PC, it works for a short time before crashing my PC? My screen simply freezes, and I have to restart my PC. When I try to use my controller on my PC, it works for a short time before freezing completely, forcing me to restart my PC.

PlayStation 4 Controller from Scuf and no it doesn't use any software
