Disconnecting Mid Game

Every game I play, I get disconnected from the servers in the 1st or 2nd period. However, I don't get disconnected from Xbox live. I feel like I've tried every solution:

- Restarted console

- Restarted WiFi

- Cleared alt. MAC addresses

- Port Forwarded 

Nothing has worked. Pls help.
