Accuracy Bug?

my character has over 100% accuracy, yet misses almost every time he attacks. do the opponents just have high defense, or do I not understand how accuracy works?

my character is lvl 37, opponent NPC are 41.

Offhand i work on the principle anything 3 levels over is a no go.
