Your graphics drivers could do with an update. I doubt it will help much but it wouldn't hurt to update them.

So back to the drawing board .... I am still testing. I guess that's what players do, really... And shouldn't have to 
Since it's seems random, it could be anything really. Especially as you said it's also happening on a fresh save which kinda rules out a save game issue.
