are you using any mods/cc in your game ?
If so, could you try without ? (you might need to try a different Sim cause removing mods may not fix already broken Sims).
If not, it's probably best if you created a bug report at the bug forum: https://answers.ea.com/t5/Bug-Reports/bd-p/The-Sims-4-Bugs
