My pet keeps disappearing

At times, my pet get's greyed out if she is not in front of me. It happens multiple times during my gameplay. When I am in front of her, she disappears in front my eyes....like poof she is gone - then she gets greyed out and I can't do any action with her. For me to get her back, I will have to Travel somewhere and include her to travel with me; then I go back home and she comes with me. But not for long, as she disappears or poofs out again.
