Frequent Game Crashes on Windows 10

I usually get less than 20 mins or so gameplay before my game crashes. After reloading the game, the game will then crash more frequently, sometimes as often as every 5-10 minutes or instantly after a reload.

The game also crashes and needs reloaded after doing certain ingame activities, more specifically when trying to do regional travel, if I leave my house and venture further the game can't handle it and crashes again.

Does anyone have any advice or help?

I have taken a copy of my dxdiag if that helps.

You have 12 GB RAM but this is wasted as you only run a 32 bit version of Windows which only support 4 GB RAM. You can upgrade to 64 bit for free by downloading it from Microsoft. Google "How to upgrade from 32 bit to 64 bit Windows 10".

Your Graphics dedicated memory is below minimum requirements for the game.
