Doctor problems in Get to work

How did you get past lvl 8 of the doctor career I can't figureout how to get a pregnant sim to come in when my sim is working. I would much appreciate some feedback please and thank you!!!

I do not get a bubble that tells me to treat patients. All I get is the usual stuff. Why can I not do anything for the doctor career. I was able to make the beds. I can't give food, take temperature, etc. Please help!!!!
