Can you let me know if these steps help you get back in?

Reboot the power to the modem/router

  1. Unplug the power cable from your modem/router for 5-10 minutes to refresh the network.

How to power cycle your Xbox console:

  1. Turn off the Xbox One console by pressing the Xbox button  on the front of the console for about 10 seconds until it shuts down completely.
  2. Unplug the console’s power cable. Wait 10 seconds.

Important Be sure to wait 10 seconds. This step resets the power supply.

  1. Plug the console power cable back in.
  2. Press the Xbox button  on your console to turn on the Xbox One.

Clear Mac Address

  1. Clear the MAC Address: From the Xbox dashboard select Settings>All Settings>Network>Network Settings>Advanced Settings>Alternate MAC address>Clear, then restart your Xbox.

I will try once more but it does not explain why my son (chrissie2003) does not have this issue and I (lvdijk67) do have in more than 50% of the times I play:

- same Xbox

- same Internet/Wifi (I have tried both Wifi and Cable)

 

Two main differences that may help you in finding the cause:

1) Mostly I encounter the problem in Squad Battles while my son is playing Rivals

2) I am using FUT App and my son doesn't  (I have tried switching off/on Verification Authorization)

 

Please check all 3-0 lost games of lvdijk67 player ID to detect how often it happens (and see also this does not happen at chrissie2003)

I had that same issue and I just fixed the problem. By going to ea.com and resetting my password. and the problem was fix just update your password. it doesn't ask you to sign back in on my game. It's just going to be fixed with a password update. From your phone or computer. It's that simple I did that before writing this and it actually worked.
