Madden 18 'Reach Out' trophy bugged? [ Edited ]

Hey, I'm on my last 3 PlayStation trophies for Madden 18 but 1 trophy in particular seems to bug out and refuses to complete it, the Reach Out trophy.

Thought maybe I was doing it wrong, searched up online and on YouTube how to achieve it, done all the steps multiple times and still no trophy?

I just want to Platinum it and be done but it's really doing my head in me. Please help.

I have 2 clips showing that I've done the reach tackle while being blocked several times if it's any use.
