My squad mates die even though they are all loyal

I have completed all loyalty missions for characters on ME2 and all characters are loyal but when I get to the cutscene after you kill the human reaper one always dies no matter who I pick in my squad. Is this a bug? As I have played it through previously and everyone lived.

Thank you everyone for your time and effort in trying to help me in this. I finally did it, Legion looked like he died but actually didn't. I changed some values in the save file with the editor, but I'm not sure if this worked or if I was being a fool the other times when I rage-quitted.

Things I have learned:

• If in doubt check your save file with the save game editor and compare against a previous playthrough (if you have one)
• Do an idiot check to make sure you're not being an idiot

Who dies? One of the squad mates who are with you or a squadmates that was holding the line? Depending on which Squadmates are left behind to defend, it is possible that one of these Squadmates can die.

E.G. Send Grunt to accompany the crew and take Garrus and Zaeed with you as Spquadmates, you may not have a sufficient defense value as these three are the strongest Squad Members.

@reidrobotThere are 3 strong defensive NPC Garrus, Grunt, and Zaeed. Never take 2 of them to final fight. You can Bring only one with you to help but the other 2 must stay and hold the line,

Initially I took Garrus and Grunt, which Tali dies. Then I tried Mordin and Jack, then Legion died. Then I tried mordin and Legion and Garrus Died

I've tried this combo also:

• Tech Specialist for the Vents: Legion

• 2nd Fire Team Leader: Miranda

• Biotic Specialist for a Shield: Samara

• Distraction Fire Team Leader: Miranda

• Normandy Crew Escort: Mordin

• Final Boss Squad: Tali and Kasumi

Garrus died this time as well

@reidrobot this is how I always play and always all survive:
Pipe work: Tali (also nice comments in ME3 about this)
Leader: Miranda / My team:Legion and Katsumi
Shield: Samara / my team: Jack and Garrus
Distraction team Miranda
Escort Thane
Final fight: Tali and Garrus

Like I said in my first reply, One strong in your team, 2 on holding possition

for the pipe u can use legion or tali.

team leaders miri or garrus

shield is samara/morinth or jack

send tali or mordin back with the crew since those two can die if they´re holding the line and use the other one for the final fight

and ur save

I've attached a picture that explains how the calculations are done for the suicide mission. That's how it worked in Legacy, anyway, maybe it can provide some help here.

Check the IGN guide. Only thing I can think off is that you picked the wrong person in one of the choices.

You can use Jacob as a team leader, is he loyal?
