ME2 Insanity Achievement Bug [PC]

How is it possible that, over a year later, the Mass Effect 2 insanity achievement is still bugged on PC? Back at launch when this bug first happened, I replayed the final mission on my old save over 10 times, and it never popped. I made a brand new insanity save a couple weeks back and just finished it this evening. Again, no achievement! I cannot believe that this is still an issue. If anyone who has managed to earn this achievement has any advice, it would be very appreciated.

I'm on Xbox and having the same issue. Just played through ME2 on insanity and nothing unlocked. I have the most up to date version too. Going through threads, it looks like BioWare patched it, but I'm still not getting it.

@ThessianBlue You're lucky, I just noticed that none of my achievements have been tracked. It has been working perfectly for ME1 but somehow entirely broken for ME2. EA desktop is not fit for purpose.
