no accept button in squad select screen

I am trying to play the game a third time. Never had this issue before.

When I get to the very first opportunity to enter squad select. Rescue the Primas for eg., the accept button flickers for a second as the screen comes up, and never appears. The only way out is alt F4. I have tried different resolutions, full screen, windowed, repair game, I even loaded the vanilla version with no dlc even though I have all of it and the deluxe edition.

I tried starting with a different mission as well. At this point only James & Liara are options. All I need to do is click accept, but the button isn't there.

Can anyone suggest a remedy for this please?

Please preform a clean boot and test the Game in clean boot mode.

If that don't helps, create a DxDiag in text file format and post it with your next reply. You can do that  with the "Choose file" button at the bottom right corner of the reply window.

Did you tried to repair the game?

(Origin > Game Library > right click ME3 > Repair / in Steam > right click at MEA > Properties > Local Files > Verify integrity of game cache)
(Steam > right click at ME3 > Properties > Local Files > Verify integrity of game cache.)
