Combat Bonus

I completed Mass Effect 3 many times and have collected all Combat Bonuses from my squad. Including all the ones from my DLC's. I've had no problem with them before whenever I start a new game and am always able to choose which one I want in character creation. However whenever I play the game now i have to restart from scratch to gain them all. Any reason this may have happened?

This might be the case already, but make sure you have all DLC installed.
