Why are my achievements not showing up?

This has been the case for many months now.

I would get an achievement (which flashes up on my screen), and then it never ever gets added to the achievements page even though I've been on and logged in for long enough.

This issue started many months ago, and never resolved itself.

Does anyone have any ideas on how to fix this?
