Xbox One - Nightsisters databank entry 5 issue

According to YouTube this databank entry is obtained while enroute to Dathomir.  It didn't unlock for me on the any of the times I went there.  I have everything else located / unlocked on Dathomir, but this.  I tried going back with Merrin as part of the crew, but that didn't work either.  I don't remember what I did while enroute to Dathomir the 1st time or on the 2nd trip back from Ilum.  I may have been in the back of the ship either at the lightsaber table, the meditation spot or the terrarium, I don't remember.

Is there another way to unlock this or is this a glitch in the game?
