I was lucky (for once in this game) & received the Tinker (Surgeon Career) Heirloom simply by opening up the green blue SUPER heirloom chest this morning after I started my night/day screen.  Good Luck to All.

*Update* ok, I have the heirloom, but cant find where to actually start career?

@OkL80Bug You have to go to the hospital and place the Surgeon table.  It should show up in the career items for the hospital. ☺
