That icon appears on non playable sims, kids and elders. It’s used to interact between playable and non playable sims.

no, it's the Good Upbringing Trait in kids, so if you interact with babies/children they will get stars for good upbringing. In elders it's the hobby or career mentoring. It's a bit of a bug at the moment, appearing in young adults, but it seems to last only a short time, just ignore it

Non playable Sims have nothing over their head, no plumbob. Other player's Sims have a blue plumbob. You can interact with both, and start relationships if you want.
