Saving appearance changes

I'm trying to change my Sims appearance but can't figure out how to save the changes!   Please help?

Depends on if you want to buy something new. If you do, you can click on the 'Checkout' and then on 'Purchase':

Or if you want to save an outfit you've already bought, or already own you can press the 'Play' button:
