Legendary MUT Rewards for several Years. I’ve never received a single reward... I can’t seem to find any answers either. I assume after 4 years of spending thousands I could get a code to redeem for my rewards. Am I missing something?

The official site to sign up for MUT Rewards can be found here: https://www.easports.com/madden-nfl/mut-rewards

One common thing we see some players run into is where they signed up for MUT Rewards with an EA account that didn't have their Gamertag/PSN ID linked to it (it was linked to a different account). If you go in game you can check to see what email address your Gamertag/PSN ID is linked to, it's worth double-checking to see if you signed up with the correct EA account.

There will be objectives based on your level that will show up in game that change every month. If you complete these different objectives, you earn rewards.
