I have the surgeon heirloom... but no new career? What else do I need...?

There is another heirloom, the medical injector or something, that is what will start the career...

I think it’s the first item listed in the pack

It should be the last heirloom on your collection.

Once you have it (you may need to place it on display in your house), you can go to the medical center and place the surgical table.  (You can then store the heirloom in storage).
