I received the 2nd code sent on Oct 18, but it was just like the 1st one, which did not work ( I was buying games at least $29.99, Portal 2, ME 3 ). And then I have not received anything after. So this survey thing really did not get me anything. I really feel like being terribly ripped off, and there's also no way to contact customer service when I run into trouble like that. Simply helpless and frustrated and disappointed.
