@gypsygirl608 Content purchased through Origin is normally added to the game library immediately. Sign into Origin, open your game library, and click on the Sims 4 icon. You should see a notification that you have new content available; it might even start downloading automatically. If it doesn't, click on More > Expansion Packs, then scroll to where Island Living is listed and click Download.
If Origin is asking you to purchase IL again, you probably just need to refresh its data. You can do so by clearing its cache (Windows) or running the Origin reset tool (macOS).
