Sims 4 origin

Ok, so I bought the sims 4 on a disc and I started playing it on origin. I didn't go on my computer for a VERRY long time so my PC reset. When I went back onto it my Origin wasn't there so I put my sims 4 disc 1 in and origin came up, I remembered I had written the login and passcode somewhere but I accidently threw it in the trash, I cant remember if I used my sisters email or mine (mine wasn't working that well so I'm not sure). I can no longer remember the username or password so I made a new account, it wont let me use the same code because I had already used it!!! WHAT  DO I DO ;"-(  mileysad:

2. Make sure you are logged in (top right corner) and select 'Help with Game'
3. Find the affected game (if it's an account issue select Origin)
4. Select 'Next' at the bottom right
5. Select your platform and then 'Next' again
6. Select topic from the drop down and type in a short description of your issue.
7. Select a contact option. Please note that available contact options do depend on your country settings and time of day.
