I need help recovering my account

@EA_Darko That form is bugged, is it not? 

Can you try submit a case and then see if you can view its contents, below the subject line? 

If you can, it would show that it's probably just something on my system. 

Afterwards, you can just delete the case, so you haven't annoyed anyone.
