First off, is your system running Windows natively, or are you using any sort of emulator or virtual machine?
Next it would be helpful if you could type out the exact error message that you receive.
7. Click the Save All Information button, found on the bottom of the DXDIAG report
8. Save this text file to your Desktop if possible
9. Open the text file, select all the text inside (Ctrl + A) and copy it (Ctrl +C).
10. Go to http://hastebin.com/, paste the contents of your DXDiag inside (Ctrl+V) and click Save on the webpage (Ctrl+S).
11. Copy the link of the page in your post, along with a precise description of your issue.

I'm having the same problem but instead of "cocoa" mine says "windows". I'm all updated and running win 7 HP. I tried everything, even did a sys file check. I downloaded the Qt for beginners app. and I have to be like a graphics engineer or something. I may know my way around many PC related stuff but there's no way I can grasp the understanding of what I am supposed to do with this Qt app. I looke4d at sys restore too, but It didn't go back far enough. Guidance needed help please, there's got to be a way around or through this.

microsoft help didn't help
reinstalling didn't help
hoping there is something because I can't access any games =(

I got these troubleshooting steps from online chat. My error message was Qt platform plugin "windows" in".
(STEP 1) For Windows 7
control panel-->appearence and personalization--->folder options--->views--->show all the hidden folders
(Step 1)
Press Windows and E at the same time.
Then click on view-->options-->views in the small window
Then select sho hidden folders and files and then, apply and ok,
(Step 2) for 7, 8 10
Please access your "C" drive.
Program data-->origin-->delete everything except Local content.
(Step 3)
Please access your "C" drive.
Users-->Your user ID folder-->App data-->Local and delete the origin folder inside it
(step 4)
Please access your "C" drive.
Users-->Your user ID folder-->App data-->roaming and delete the origin folder inside it
now please restart the PC
I'm not sure what is meant by local content - if anyone could help me with that

I've been having the same problem as you but with windows not cocoa. I have just uninstalled Origin with Revo and reinstalled and it's working.
Just in case you needed this.

If you're still having problems try uninstalling Origin with Revo and then reinstall. It has worked for me. =))
