We are tracking some issues currently affecting Dragon Age: Inquisition. Many of these have workarounds or solutions, so please check below to see if any of these describe your issue:

Here a few tips to help circumvent the issue while the Dragon Age team is working on resolving it:
Do not alter the command line in the game's properties with any unofficial workarounds. Doing so may prevent the game from performing as it should. Next be sure to download the latest drivers for your graphics card and turn down your graphical settings.

With the release of Windows 10, we see more issues related to graphics drivers. It is recommended to completely remove your Windows 7/8 graphics drivers and install the latest Windows 10 drivers from the website of your GPU manufacturer.
To perform a clean install, go to the control panel by right clicking the start menu button and clicking 'control panel'. Then go to 'uninstall a program' and uninstall all software from either Nvidia or AMD (depending on what you have).
Then install the latest Windows 10 drivers for your GPU.
