Herald Achievement not unlocking PC

I just finished a complete game on hard difficulty without changing to a lower difficulty at any time during the game. However, I DID NOT receive the Herald achievement when I should have. I'm playing on PC and have had no trouble with the achievements before. All flagged for me at the appropriate times.

My understanding is that the difficulty achievements only apply to the actual game content as released in November 2014 and do not apply to the Jaws of Hakkon DLC. Is this true or do I need to open up the save and play JOH to get the achievement?

Just an update to close this question:

I never could get the particular game I was playing to flag the Herald achievement so I started another one. I played on nightmare and managed to slog my way through a bare bones playthrough that wasn't pretty to watch but worked. I received the Herald and Inquisitor achievements with no problem so I now have them all.

It's not a solution to the problem but I'll mark it as one to close the question. Thanks to all those who replied!

I don't think JoH is a factor, no. 

Thanks for the post, perhaps the issue is related to Origin itself? By any chance is Origin In-Game disabled?

I do have Origin in game disabled and don't log into DAI in game either. Since I posted this, I log on in game but don't have Origin enabled. I was using a low end GPU until recently and never bothered to re-enable Origin in game. I'll try turning on Origin in game later and come back with an update. Thanks for the reply EA_David!

I fired up that particular game save, making sure I had Origin in game enabled. I logged into the DAI servers within the game also. So after visiting the Black Emporium and running through the Hinterlands killing defenseless wildlife for a bit, I still don't have the Herald Achievement. I'm guessing there's a glitch somewhere. I've never changed the difficulty or adjusted any game play settings. The only thing I haven't done is back up to before I killed Corypheus and kill him again or go play the Jaws of Hakkon and see if that triggers the achievement.
