An update: I found the problem with my system and resolved it. My video card was overheating. I resolved the problem by turning down the various graphics settings until everything stabilized and my GPU was constantly running at 60 F or cooler.

This to me looks like a bug; the full text from my device is

DirectX function "GetDeviceRemovedReason" failed with

DXGI_ERROR_DEVICE_HUNG ("The application's device failed due to

badly formed commands sent by the application. This is an

design-time issue that should be investigated and fixed. "). GPU:

"NVIDIA GeForce GTX 660 ", Driver 34475

I've always seen this error during cutscenes, never during gameplay, so I'm fairly certain the bug is there.

Edit - I'm running Windows 8.1 64 bit with latest updates and driver. I HIGHLY suspect Windows 8.1 because I've not had the crash on my Windows 7 64 bit laptop (though I also am nowhere near as far into the game, either).

I have exactly the same suspicion as ClewinX. And I also have a GeForce GTX 660 with the latest driver.

My game ONLY crash during cutscenes. Never during normal play.

Here's some examples of cutscene crashes:

When I had to fight Corypheus.
When I travelled to Skyhold (exactly when the dialogue with Solas ended).
When I met my Hawke from DA2 on the castle wall.
When I try to judge on the throne.

The only way I can get pass these crashes is by rapidly tapping on Skip/Escape. I guess in this way I cancel some processes/scripts? Or maybe I just spare the graphics card for some work (same as going down to Low settings)?

Is the graphics card overheated or overburdened somehow? What is wrong with these cutscenes?

It seems logical that there are either some bad commands that causes a conflict or maybe a setting that doesn't fit the rest?
A process ends and something else is taking over. This is the point where the error occours.

This fixed the crashing for me:

- Go to your registry: Start --> Run --> Type in "regedit"

- Go to "HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\GraphicsDrivers" and create a key of type "DWORD (32-bit)" and name "TdrDelay"        with a value of 10 as Decimal value.

- Reboot computer.

Unfortunately, it didn't help the framerate issues.

I don't crash as often, but I still crash (Nvidia).

Windows 7 I assume? Yes I had the same problem. game ran fine until then but a rollback did not fix the issue and this new patch has not resolved it.

It was not.  I do not believe it was addressed at all by the patch.  The only reference to this issue I have seen from Bioware is that they are working with Nvidia on the problem.  That was back on November 18th.  There have been no updates on the issue since then, including whether the fix will be a patch from Bioware or new drivers from Nvidia.  I doubt we will receive any updates on progress until right before they release anything (if they ever do.)
