After loading new worldslate cant start game

For some reason when I load a new worldslate and try to create a new character my game does not start.  

Here is how it goes mileyfrustrated: 

I start the game after my character wakes up in the Fade I do the customization, the cutscene ends I end up with extremely low health in the same place with nothing and nobody around me and no exit.  I never get the part where Cassandara and Leliana questions the inquisitor.  

I play as a elf (class does not matter)
