Sometimes it is possible to shoot just over the edge of a wall and still hit your target . Here it seems to be the same case as units moving on the balcony that do not kneel or crawl are visible just enough with their head to get it shot off by any gun aiming at you if you are in that position. Another problem could be when the wall is mostly destroyed and should appear so to all players only to find out the animation or state of the building is not there to show for it. Also exploding bullets do not care if you are hiding or not they will just damage anything close enough to the impact point of the bullet.
The game has functioned like this since day one and is I think not considered a major bug i doubt they will ever be able to properly address this without spending more than a little time on it.
