Also had the same issue. (Since launch).
Was using GTX 770 first, had the lines. Upgraded to GTX 970 later, lines were even worse.
Then I got so fed up, so I switched to AMD R9 390 and the lines are gone. However, I want to switch to Nvidia since some other games I play are having issues with AMD cards.
This is a typical Nvidia problem, since all the drivers and everything were up-to-date.
I hope they can fix this ''soon'' because this issue is annoying.
Also turning off Edge Smoothing does get rid of the lines, but it doesn't make it better, since obviously the edges in game are horrible that way.

Having the same issue. Also using an Nvidia card.
The sims 4 has so many graphical problems with Nvidia cards it's laughable.
I can't believe this problem hasn't been addressed yet. It's been reported about since 2014. Does anybody at EA actually care at all?
This in conjunction with the shadow bug that is also plaguing Nvidia users is really hurting the game experience. I wonder what other bugs I'll find if I continue playing :/

This works for me. All you have to do is download the NVIDIA Inspector and tweak it's settings or turn Antialiasing Fix on for The Sims 4.
I might make a video instruction about it for those who doesn't get it.
Here's a video that I made for the fix. (Only for NVIDIA users)

PS: I use a GTX 970 and play on Ultra Settings.

I'm having the same issue too. Looking at the date, it seems to be an older bug, any idea why it hasn't been fixed yet?

It's a bug caused by the graphics card and not by the game itself. So don't ask why this bug is not fixed yet because not all of us have encountered this bug.
Anyway here's the fix:

It uses a third-party program but it's safe. You just need to activate the Anti-Aliasing Fix.
PS: It's for NVIDIA users only.

While in create a sim mode, I have these black thin lines that go around the mouth and cheeks, they are super thin. I tried to change the grapgics settings, and found out that they disapear if you turn of antialiasing, the setting that makes lines smoother, so it has something to do with that?
Is anyone lese having this issue? Is there a fix for it?
I have updated my driver and have recomended specs for the game, Invidia.
