The last three days I have tried everything. Nothing worked. I realized this is a bit unconventional and a lot of people may not want the solution. It had been almost 2 years since I've done a clean install of Windows on my game machine.
So that's exactly what I did. I wiped the drive and did a clean install of Windows 10. I then installed Geforce experience&drivers. Oculus software, and then origin. Downloaded squadrons. Rebooted the machine. And fired everything up. Those are the only four things I put on the computer at the moment. And VR works as it should every time I log in. I've actually managed to get all the way through the prologue. Everything is cranked on ultra and runs okay. The joystick dead zone is still an issue. But I never had a crash.
My take away from this? No matter what I thought I had on my computer. I clearly had something installed that was conflicting with Oculus and origin causing the crash before. I may have installed something and uninstalled it leaving some remnant behind at one point in the last few years. Who knows.

What happens when the bug occurs? Every time the game is quit, it crashes SteamVR which then must be restarted.
What do you expect to see? I expect SteamVR to not crash...

Please fix so SteamVR doesn't crash every time I exit the game.

I can also confirm the exact same thing.
Crashes to desktop during the following...
Anytime I try to load into a Fleet Battle. Either training or Ranked Multiplayer.
Not having problems with anything else.

OMG OMG! Thank you so much. This worked for me as well.
Just to be clear, the fix for this is the following...
Launch the game in VR
Go into Video settings
Scroll down to "VR Quality" and set it to "Custom"
Scroll down further until you find "VR Volumetric"
Set that to Medium.
Finally enjoy the full game in VR. THANKS SO MUCH.

Same issue. Crashes on opening load and victory. Also affects multiplayer matches and I question if it is specifically the Yavin map used in that tutorial.
Did all the same steps including changing in game settings to low (including Volumetric) and VR SS and settings lower. It behaves the same.
I also get severe ghosting and seemingly frame skipping when moving with the stick, but not at all when moving my head to render the same things such as in the hanger.

Confirmed the problem was resolved for me (Valve Index, crashing on load/death) after reducing Volumetric Quality down to Medium

@redhotyamada was right !!!
There are some grapics settings for VR and others for classic gaming.
I did change the volumetric quality to low in the VR section this time (until then I was chaning the settings for screen gaming) and I don't have any crash on Yavin anymore !
Just did the Battle Fleet tutorial and everything worked just fine

I am still having issues with the transition in fleet battle training. but all other transitions for vr are fine.
