Steps: How can we find the bug ourselves? 1. Have a hosehold which contains 2+ adult sims. 2. In live mode, open relationship panel. 3. right-side: Click heart icon. It will turn green. 4. left-side: Click other sim.
What happens when the bug occurs? 5. The green heart icon become gray - it is still selected, though.
What do you expect to see? 5. The heart icon stays green. Other icons such as coworker icon don't have this problem.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
