The "SQUAD ELMINATED" screen only shows 2 out of 3 squad members

Where did the issue occur? Returning to the lobby after a match
Which part of the map or menu were you in? If you don't remember the exact name, please describe the area or what you were trying to do in the menu. the match ended at hydrodam, but I doubt it has anything to do with this problem
What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. the "squad eliminated" screen was displayed
Did your squad mates also experience the issue? I don't know

How often does the bug occur? Rarely (0% - 9%)
How severe is your issue? Little to no impact to gameplay
What happens when the bug occurs? The "SQUAD ELIMINATED" screen is displayed, and it only shows 2 out of 3 squad members.
What should be happening instead? It should show all 3 squad members
Steps: How can we find the bug ourselves? Impossible for me to say.

Do you know if that 3rd player missing from the squad eliminated screen shut off their system just before the squad was wiped?

I checked the recording, and everyone was present when the squad was wiped. I knew that this one would probably be hard to figure out...
