Sith Trio Raids Missing after Claim

What type of issue do you have? Missing Rewards
How often does the bug occur? Occasionally (10% - 49%)
Summarize your bug At least partial claimed rewards for trio raid not placed in inventory for the second time. I only checked the completed medpacs.
Steps: How can we find the bug ourselves? Hopefully there is a log of rewards vs my inventory. Otherwise, this is probably not provable.

Guild finished Sith Trio Raid (not heroic, but the one below it). Rewards showed that I received 2 completed Mk 6 Athakam Medpac's along with a ton of other stuff. I only paid attention to the medpacs. I claimed the rewards and went to use them/check other gear needed on Bossk, but they are not showing in my inventory. This happened a few weeks ago, and I assumed it was my mistake. I did NOT make the same mistake this time around.

The only other thing I can imagine is that it was a visual bug falsely showing that as a reward. Hopefully that's not the case.
