Steps: How can we find the bug ourselves? Attack an Octane, and when he has 10 or less hp, use his tactical ability at the same time as being damaged.
After getting damaged by an enemy, I used my tactical ability, Stim and was knocked by it (which isn't supposed to kill the character), I've attached the screenshot of the death recap screen.
