Also having the same issue, finished making a manor for my new vampire family and the sim I sent to the sauna ended up wearing the towel, yellow socks and the astronaut helmet.............
- Changing into a towel at the Gym or Spa will no longer cause a Sim to occasionally wear additional accessories.
- After a long, hard day of beekeeping (Requires The Sims 4 Seasons), Sims will now fully remove their beekeeper suit when heading to the sauna or massage table.
Fixed in Patch 1.47.49 .. or not

Is this an existing game ? They might still be wearing it from before. Not sure if the fix fixes existing bad outfits.

Yep, it seems this bug managed to get UNfixed.
The hat is probably supposed to be there, as its part of her everyday outfit, but the gloves, bracelets, and those leggings are not part of any of that or any of her outfits.
This bug is still going strong.
