I've asked myself the same question many times. I remember a table in the Movie expansion pack that wouldn't show a pattern correctly, and I think it took them years to fix that bug - if they ever did.
I don't play with mods and it happens to me. So, I thought maybe my Sims kept their coats on because it was cold inside the house and I forgot to set the thermostat, but that has nothing to do with it. If I take them to an outside lot and they wear party clothes, they won't change into their winter clothes while outside. They'll shiver and freeze to death outside the restaurant unless I tell them to put on their outdoor clothing. The other sims that I have no control over wear their party clothes outside and also freeze to death on these lots. They really need to do something about fixing that. I wonder if it has anything to do with how you can make outdoor outfits your main outfits.

I also have a problem with this. I've had to turn off temperature effects. I can change my household Sims into appropriate clothing before leaving the house, and then they switch to everyday clothes before leaving for work or school. About half the NPC Sims wear appropriate clothing and half don't. If we travel somewhere outdoors, I have to manually make my Sims change clothes and again, a lot of NPCs are in inappropriate clothing for the temperature.

For anyone still encountering this (without mods), some questions:
Note: If you change the outfit of your active Sim manually, it is by design that the Sim does not automatically change outfit the next time the Sim goes outdoors/indoors. If the Sim never automatically changes some time after that though, it is an issue.

After the latest patch my sims won't even change into their everyday outfit before going to school anymore... now they just go in their pjs if I don't change it, and they never put on weather appropriate clothing either. Very frustrating.
