I have the same issue, but not all the time. I tried to take cc-hair of the sim and didn't help. I used the sliders to tone down her makeup. Looks fine in CAS, but in game, the sliders have no effect and her makeup looks crazy. Any solution to this ?

Steps: How can we find the bug ourselves? Use slider custom lipstick and leave CAS
What happens when the bug occurs? Whenever a Sim is wearing makeup customized with sliders the color will return to the default once I leave CAS. It always happens with the lipstick, the eyeshadow will -sometimes- remain the same. Sometimes, when I return to CAS, it goes back to the customized color.
What do you expect to see? The color to stay customized outside of CAS
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure
I doesn't seem to happen with all the lipsticks, this Sim has a Vampires lipstick in her Formal and it has not changed, but it definitely happens every time with the matte base game lipstick as well as the MAC lip stain.

Lately I've had the issue where my Sims' makeup and sometimes skin tone that I set in CAS looks completely different in game. This happens a lot with custom makeup swatches - they do not translate outside of CAS, so they end up with ridiculous looking makeup in-game.
Recently I downloaded a Sim from the gallery, and when I started playing her in live mode, her skin tone was several shades darker than what it shows in CAS. I noticed that it's a custom skin tone, so my theory is that customized makeup and skin isn't showing up correctly in gameplay.
Any solutions?

I had this issue with makeup. I put eye shadow on a sim, used the sliders to turn down the opacity a lot, and then when I started playing, the eye shadow was the default opacity.
