More instances of 'No Hit Reg' and an Octane Stim bug

Where did the issue occur? In a match
Which part of the map or menu were you in? If you don't remember the exact name, please describe the area or what you were trying to do in the menu. Near the Bridges running uphill towards the Hydro Dam
What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. Shooting and enemy and running while stimmed and trying to ult
Did your squad mates also experience the issue? I don't know
How many matches had you played in a row before the issue occurred? About 4 or 5

How often does the bug occur? Often (50% - 99%)
How severe is your issue?
What happens when the bug occurs? For the first bug, no damage registration and for the second bug, I couldn't do any actions until my Stim ended.
What should be happening instead? damage numbers popping out and my ult being thrown out
Steps: How can we find the bug ourselves? Shoot and enemy and try doing things just after you stim or as it's ending.

I was testing some new equipment in my stream today and noticed some instances where my shots didn't register damage.

In both examples, you can hear my bullets hit the target and see some blood splatter but no damage numbers.

And bonus, my Jump Pad didn't get thrown just as I activated my Stim. I couldn't really do anything as well until the buff ended (Seems like other people have had this bug as well).
