How often does the bug occur? Every time (100%)
Summarize your bug My guild did Hoth: rebel assault territory battle that ended at 10am on September 11th.  I was kicked from our guild just after 10 to allow a bigger player in our alliance to come in and help with cpit and I joined a different guild in our alliance.  We got 38 stars and I participated in every phase, but I did not.get the final rewards and EA help just tells me I am not eligible.
Steps: How can we find the bug ourselves?

Based on our logs, you did not participate in TB: Hoth: Rebel Assault. None of the two guilds have your account on their lists. As already explained, you are not eligible for the rewards.

If I didn't participate then how did I get special mission rewards in phase 5?

Yep, you guys are just as useless as EA help.  Good job solving this problem!!  Ignoring things is much easier than trying to fix something.
